- Only one entry per person, per day is permitted.
- All methods of entry have an equal chance of winning.
- The code printed on your receipt can only be used once.
- Survey is only open to legal residents of Iowa, Missouri, South Dakota, Kansas, Illinois, Nebraska, Minnesota and Wisconsin.
- Prizes are not redeemable for cash.
- Prizes are non-assignable or transferable.
- No employees, officers, sponsors and their subsidiaries of Hy-Vee are eligible.

Step 1 Visit the www.hy-veesurvey.com and Click on the button”Take Survey” to Begin the survey.
Step 2 Enter the 16 digit survey code and store number from your survey receipt.
Step 3 Once you’ve supplied the code you’ll be taken to the questionnaire. The questions are going to center on your most recent experience at Hy-Vee.
Step 4 Answer all the questions based on the recent experience in the Hy-Vee. Answer as genuinely as possible & click Submit Button.
Step 5 Upon completion of the survey, the participant will be invited to complete the entry form in its entirety and submit to receive one Sweepstakes entry for the month during which the entry is received (the “Survey Entry”).
To enter the Sweepstakes without making a purchase at a Hy-Vee or without completing the survey, hand print your name, complete address, city, state, zip code, daytime phone number, and birth date on a 3” x 5” card and mail it to: Hy-Vee Customer Experience Survey Sweepstakes Entry, 625 Panorama Trail, Suite 2100, Rochester, NY 14625-2437. Any mailed Sweepstakes Entry must be postmarked on a date that is within the Sweepstakes Period to be eligible.
WINNER LIST: For the names of winners, send a stamped, self-addressed envelope to: Hy-Vee Customer Experience Survey Sweepstakes Winners, P.O. Box 25466, Rochester, NY 14625-0466. Sponsor must receive all requests for the names of winners no later than 30 days after the published end date of the Sweepstakes.
